Reporting a Critical Incident
To report or discuss a critical incident, 24 hours a day, please call:
- 8 AM – 4 PM: 926.7166 or 232.0824
- After hours: DSM Administrator on-call pager – 204.935.2759
Critical incidents are unintended events that occur when health services are provided to an individual, resulting in a consequence to the patient that:
- is serious and undesired, such as death, disability, injury or harm, unplanned admission to hospital or unusual extension of hospital stay, and
- does not result from the individual’s underlying health condition or from a risk inherent in providing the health services.
If you or a family member are involved in a critical incident related to DSM’s diagnostic care, DSM wishes to support you through this process. DSM does encourage you to call but also understands it is a difficult decision to make. We respect your privacy by offering anonymous reporting, as well as any additional support that may be needed in helping you to deal with a difficult situation.
DSM’s critical incident processes are established to improve the quality and safety of our services. The details you share about a critical incident will contribute to its investigation and to making systemic improvements that will lead to safer patient care.
